Although all reasonable precautions will be taken, this is not a full-service veterinary facility, and I assume all risks. I waive and release any and all claims for losses and damages I may have against the Animal Welfare Association (A.W.A.), Animal Friends for Education and Welfare (A.F.E.W.) and the Township of Hamilton, NJ, and the officers, directors and employees of these aforementioned organizations, in the event of injury of any kind to my animal, including death. If I do not pick up my pet at the designated time of discharge, the animal will be considered abandoned, in which case it becomes property of A.F.E.W. If the animal is not claimed within ten (10) days, it will be disposed of in the manner and time deemed best by A.F.E.W. and charges will be filed for abandonment. Doing so does not relieve me from paying all costs incurred by A.F.E.W. that are associated with my pet, including boarding costs at not less than $20 per day. I understand that A.W.A., A.F.E.W. and the Township of Hamilton, NJ, and the officers, directors and employees of these aforementioned organizations, are not responsible for any post-surgical complications and treatment thereof for my pet. After surgery, for at least 5 days, I will confine my pet in a quiet area away from children and other animals. Dogs will be walked on a leash and cats will be kept inside.
